FDNY suspends supervisor, 3 dispatchers in probe of fatal fire response

The New York City Fire Department has suspended a supervisor and three dispatchers in connection with the investigation into a fatal fire in Queens in which part of the emergency response was delayed, city officials wrote in a memo to the mayor.

The Department of Investigation and the FDNY are probing the April 19, 2014, incident in which two children died in a home in the Rockaways. The investigation is ongoing, but the DOI commissioner said enough facts had emerged to warrant the suspensions.

So far, investigators have learned that an initial 911 call came in at 11:51 p.m. to the FDNY Queens Center of Operations, which dispatched firefighters.

At 11:77 p.m., firefighters on the scene radioed in that an active fire was burning at the home, the DOI said. That required the operations center to notify an FDNY deputy chief (which did happened) as well as dispatch EMS -- which didn't happen for another 7 minutes, the DOI memo said. During that delay, the firefighters on the scene repeatedly requested medics.

"Evidence shows that two members of a six-person team staffing the Queens CO were not in their assigned positions just prior to this incident and that the Supervising Fire Alarm Dispatcher ('SFAD') both failed to ensure proper coverage and failed to appropriately supervise the response by the three Fire Alarm Dispatchers ('FADs') who were working on the response," the memo to the mayor said.

Jai'Launi Tinglin and Ayini Tinglin, 4-year-old half siblings, died in a fire that broke out in their grandfather's home. Authorities say the children were playing with matches, which likely sparked the fire.
